Evaluation & Report Due Dates

Unit-1: 2008-2009 Program Year Reports We are only collecting Data on the subject matter contained in Unit 1 over the next two year period. County Educators are encourage to continue use of materials from Units 2 and 3 as needed but to put emphasis on the content of Unit 1
- Evaluation Instructions Jan. 2008
- Due: February 1 to District Program Specialist
  - Form 1B - County Extension Educator's Observations of Recognizable Changes in Competency Levels
  - Form 1A - Six month Evaluation of Material Use
- Due: August 1 to District Program Specialist
Unit-2: 2003-2005 Program Reports
- Due: February 1 to District Program Specialist
  - Form 2B - County Extension Educator's Observations of Recognizable Changes in Competency Levels
  - Form 2A - Six month Evaluation of Material Use
- Due: August 1 to District Program Specialist
  - Form 2B - County Extension Educator's Observations of Recognizable Changes in Competency Levels
  - Form 2A - Six month Evaluation of Material Use
  - Form 2E Volunteer End of Year Evaluation
    or
  - Form 2C Pre-Volunteer: Beginning of Year Evaluation (Pre test must be accompanied by Post test)
    and
  - Form 2D Post-Volunteer: End of Year Evaluation (Post test must be accompanied by Pre-test)
